What is a Aging Life Care Manager & Do You Need One?

All of us are navigating our aging journeys, no matter our age! Families are often faced with difficult questions about care options for loved ones, changing needs, and how to balance independence with safety. Aging can bring medical, emotional, logistical, and financial challenges not only for older adults but also for spouses, adult children, and caregivers. Each family is unique in addressing the issues. We’re here to help.

As part of Unity’s Life Planning Series about aging-in-place resources, this free community forum will introduce the role of Aging Life Care Manager and how they differ from senior home care agencies or hospital case managers. The panelists will respond to questions about their role and how proactive planning can help prevent crises, improve communication among care providers and families, and support better outcomes for older adults.

Meet The Experts

Meike Wiest, MSW, CMC, NCG is a Care Management Supervisor with Arosa, a company that helps seniors age at home with comfort and dignity. They develop custom care plans, in home care, and concierge nursing at home.
Trish Cook is the owner of Cook Care Management. She has an MS in Gerontology and is a Care Manager Certified (CMC). They provide Needs Assessment and Care plans, Family Support, Crisis Intervention, Advocacy, Housing, Cognitive Support, Referrals to local resources, financial and legal review. Cook Care Management also specializes in dementia and Alzheimer’s care and resources.
Lynne Moore, RN, CMC, CAPS is executive director of ThriveMore at Home, a CCRC (Continuing Care Retirement Center) at home. A fairly new concept to help seniors go from independent living to assisted living to nursing home care.
