All of us are navigating our aging journeys, no matter our age! Families are often faced with difficult questions about care options for loved ones, changing needs, and how to balance independence with safety. Aging can bring medical, emotional, logistical, and financial challenges not only for older adults but also for spouses, adult children, and caregivers. Each family is unique in addressing the issues. We’re here to help.
As part of Unity’s Life Planning Series about aging-in-place resources, this free community forum will introduce the role of Aging Life Care Manager and how they differ from senior home care agencies or hospital case managers. The panelists will respond to questions about their role and how proactive planning can help prevent crises, improve communication among care providers and families, and support better outcomes for older adults.
